Billing Policies

1. Self-pay patients are expected to pay at the time of service
   
   
2. Co-pays are due at the time of service
   
   
3. Your insurance cards need to be presented at each appointment
   
   
4. We will submit to most insurance companies directly, then any remaining balance will be due by the patient
   
   
5. We accept cash, check, Mastercard, Visa and Discover
   
   
6. There will be an $20.00 fee applied for returned checks
