Appointments
The Center is open Monday through Friday 8:00 am to 4:00 pm. Please call us at 585-256-3550. Emergencies are handled through our professional answering service at 585-258-4840.
Patients are seen by appointment only or in a serious emergency. If it is necessary to cancel an established appointment, we appreciate 24 hours notice.
Completion of patient forms will be required on the initial visit, including a list of current medications, treatments and prior hospitalizations. Please print these patient forms, fill them out and bring them with you to your appointment or fax them to 585-256-3554.

BILLING POLICIES
Self-pay patients are expected to pay at the time of service.
Co-pays are due at the time of service.
Most insurance plans are accepted.
Your insurance cards need to be presented at each appointment.
We will submit to most insurance companies directly, then any remaining balance will be due by the patient.
We accept cash, check, MasterCard, Visa and Discover.
There will be a $20 fee for returned checks.
There will be a $10 processing fee for completion of disability forms.
